Out of network & self-pay patients:

 Cultivate is proud to be in network with most commercial insurance plans.  If your insurance is not accepted at Cultivate Psychiatry  & Wellness or you do not have insurance,  you can engage in care with Cultivate as a "self-pay" patient. For more information on our fee schedule for self-pay patients please visit our fees page, here.

Cultivate is happy to offer our commercially insured self-pay patients a superbill to submit to your insurance plan for out of network reimbursement. Self-pay fees are payable via cash, card, FSA, or HSA funds and are due at time of service. Please note, superbills cannot be provided for any Medicaid product or plan (Buckeye, Caresource, etc.,).
