Fees

Cultivate is in network with many commercial insurances companies and will bill your insurance for most services. Our fee schedule for self-pay patients and services not billed to insurances is listed below.

Medication Management Self-Pay Rate:

  • Initial Appointment: $300
  • Follow Up: $175


Therapy Services Self-Pay Rate:

  • Initial Session: $150
  • Ongoing Psychotherapy Session: $150


Medical Marijuana Examination & Recommendation Self-Pay Rate :

  • Initial Evaluation: $100
  • Yearly Recertification: $75

Please Note:

Cultivate Psychiatry and Wellness does not offer services for completing disability paperwork, including Social Security Disability (SSD) forms or offer disability assessments. We do not guarantee that our clinicians will complete any such paperwork, including FMLA/STD etc., unless legally required. We appreciate your understanding.



Good Faith Estimate:

  • By law, healthcare practices must provide a Good Faith Estimate of expected charges for medical services to self-pay and uninsured patients. You have the right to receive a Good Faith Estimate explaining how much your medical care will cost
  • For psychiatry appointments: the maximum out of pocket cost for a new or established medication management appointment is $550.
  • For psychotherapy appointments: the maximum out of pocket cost for a new or established counseling appointment is $300
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you an dispute the bill
